An MP3 is a sequence of frames, each holding a fixed number of samples and decodable on its own. Cutting on a frame boundary means the kept frames can be copied byte-for-byte into the new file, so nothing is decoded or re-encoded and the audio you keep is identical to the source.
Frames are about 26 ms long, so a lossless cut snaps to the nearest boundary. The result panel reports how far it moved. If you need the exact sample, switch to Precise mode.
Two things, stated plainly. Cuts snap to frame boundaries, and MP3 frames may borrow data from the couple of frames before them, so the first moments after a cut can sound slightly thin. Precise mode avoids both by decoding first.
Because writing MP3 again would be the second lossy pass this tool exists to avoid. WAV keeps the decoded signal intact; it is simply larger.
Lossless mode is for MP3. Precise mode accepts whatever your browser can decode — commonly WAV, M4A/AAC, OGG and FLAC alongside MP3.
